はり・きゅう、あんま・マッサージ・指圧の施術については、一定の要件を満たす場合、健康保険（療養費）の支給対象となります。

提出期限 すみやかに
対象者 療養費の支給要件に該当し、保険医の診察・同意書の交付を受けた被保険者、被扶養者

備考 6ヵ月を超えて施術が必要な場合（変形徒手矯正術については1ヵ月）、再度、保険医の同意書が必要となります。
